Reflexiones sobre una crisis que no cesa. Un rayo de esperanza.

Una crisis estructural

Creo que todos coincidimos en que padecemos una crisis económica. Es evidente. Pero esta crisis no es como las otras. No es cíclica, es estructural. Viene acompañada por una crisis de los valores, una crisis política, una crisis demográfica y un cambio geopolítico y geoeconómico en un nuevo mundo, el llamado mundo global.

Desde Europa hemos llevado nuestra cultura, nuestra civilización de progreso y libertad a gran parte del mundo pero ahora llevamos años cuestionándola de manera irresponsable en nuestro propios países, al tiempo que nos hemos dejado caer en la indolencia y la comodidad creyendo que nos lo podíamos permitir.

Nuestra crisis es estructural. Necesitamos un cambio en nuestro modelo de comportamiento, un cambio del modelo político y legislativo, judicial, económico y laboral; un cambio del modelo educativo, sanitario y de servicios sociales.

La crisis económica en España

Pero centrémonos en lo inmediato, en el aquí y ahora. El hecho cierto es que en España tenemos menos dinero que antes, ganamos menos, y ante esta situación, una persona sensata y responsable solo puede hacer tres cosas:

  • Gastar en menos cosas evitando gastos superfluos eligiendo lo más importante
  • Comprar más barato, buscando aunque incomode realizar esa búsqueda
  • Buscar la manera de ganar más, de aumentar los ingresos, trabajando más o trabajando mejor

Ganamos menos como país porque, al ganar menos las empresas y los trabajadores, muchos de los cuales no ganan nada porque están en el paro, hay menos ingresos para el Estado procedentes de los impuestos sobre la renta de los trabajadores y de las empresas y, al haber menos consumo, de los ingresos procedentes del IVA. Además, el subsidio de desempleo de los trabajadores supone un coste adicional para las arcas públicas.

Como el Estado no hizo lo que un buen padre de familia haría y siguió gastando, tiene ahora una deuda importante que le cuesta muchos intereses y que ha de devolver. Por ello el nuevo gobierno se ha visto obligado a subir los impuestos, pero eso no dará para cambiar la situación, incluso puede agravarla.

La crisis demográfica

El envejecimiento provocará, está ya provocando, un aumento del gasto sanitario. Si a esto le unimos ya no el descenso sino la falta de natalidad, provocará la insostenibilidad matemática del sistema de pensiones y una disminución demográfica que necesariamente conllevará una depresión económica permanente. Un problema que afecta a toda Europa y, de manera grave a países como Alemania, España e Italia.

Las medidas que se puedan tomar llegan tarde pero no por ello deben dejarse de adoptar. Vivimos en una nube desde hace años sin darnos cuenta de lo que nos está pasando.

La actitud de los políticos, en busca del voto, ha sido la de ofrecer el oro y el moro: servicios sociales, vacaciones, viviendas para jóvenes, becas y mas becas sin control, servicios sanitarios, centros y más centros de salud, hospitales y mas hospitales y en ocasiones cuanto más grandes mejor, en una época de aparentes vacas gordas y compitiendo entre las 17 Comunidades Autónomas a ver quién da más.

La crisis financiera

La crisis financiera en España tiene tres componentes: i) la derivada de los activos tóxicos, en su mayoría provenientes de los Estados Unidos, en los que invirtieron muchas de las Cajas de Ahorros, ii) la derivada de una política de inversiones irresponsable realizada por las Cajas de Ahorros, mediatizada por los gobiernos autonómicos y iii) la derivada de una descontrolada concesión de créditos e hipotecas para la compra de viviendas e inmuebles sin garantías suficientes en un mercado súper recalentado.

Nuestra situación, que también es la de otros países de Europa, se agravó respecto a estos con la burbuja inmobiliaria, engordada por una banca sin control que ofrecía dinero sin garantías suficiente de manera irresponsable y que ahora hacen todo lo contrario: no lo dan, pasando así de un extremo a otro sin que las empresas tuvieran tiempo para adaptarse a la nueva situación. Todo ello ante un gobierno que, por motivos electorales y probablemente también por incompetencia, se negaba a reconocer la llegada y gravedad de la crisis y, por tanto, no tomó medidas que pudieran paliarla al menos en parte, que hubieran situado a nuestra nación en una posición de fortaleza respecto a otros países de nuestro entorno.

Para que nuestro sistema financiero, nuestros bancos, puedan proveer del crédito a las empresas para financiar su fondo de maniobra permitiéndolas así desarrollar sus negocios; para evitar acciones especulativas o acaparadoras en nuestro sistema financiero, es muy importante que el Gobierno acometa cuanto antes la reforma del sistema bancario. En Europa se han acometido medidas para proveer de fondos a los bancos con el fin de que éstos puedan proveer de dinero a las empresas pero en España esto no ocurre; A pesar de que los bancos españoles pueden acceder a este dinero barato ofrecido por el Banco Central
Europeo, las empresas siguen sin tener la necesaria accesibilidad a los créditos.

El cambio necesario

Los ciudadanos esperamos medidas ejemplares por parte de los políticos. Ansiábamos el cambio ante el cinismo de los políticos que detectaban el poder, la mentira impune, la corrupción de sus dirigentes, la manipulación de la justicia, el ataque a la oposición como única estrategia, la ignorancia e incapacidad de los dirigentes, la Corrupción de Estado repartiendo dádivas y favores a fundaciones y organizaciones de dentro y de fuera, la presión continua contra el ciudadano.

El ciudadano ha sido tratado como un presunto delincuente, sometido a múltiples controles, investigaciones, multas (¡porque te vamos a pillar…! ¿Recuerdan?), mientras ve que asesinos y delincuentes salen a la calle apenas sin cumplir condena.

El ciudadano es el que soporta el ajuste la crisis yéndose al paro, cerrando las pequeñas y medianas empresas, mientras los directivos de los Bancos se forran llevándose un motón de dinero de Cajas de Ahorro o Bancos en crisis.

Los ciudadanos reclaman medidas ejemplares de la clase política y bancaria. Pero también reclaman la exigencia de responsabilidades a aquellos dirigentes que actuaron, cuando no dolosa, al menos, irresponsablemente.

Es necesario que las medidas de austeridad se prioricen en las estructuras políticas de las Administraciones Públicas y en aquellas que tengan menos impacto en los servicios sociales, especialmente en aquellos que sean más necesarios. Las autoridades políticas y de los gobiernos tienen que dar ejemplo de austeridad y deben comunicar a los ciudadanos porque se toman estas medidas de austeridad y quienes fueron los que crearon esta situación.

Igualmente hay que estudiar el impacto en el empleo pero, sobre todo, el impacto en el tejido empresarial e industria que pueda tener estas medidas de ajuste del gasto público, de manera que los daños colaterales sean lo menores posibles. Destruir el tejido industrial es fácil pero crearlo sabemos que es muy difícil. Por ello estas medidas deberían ir acompañadas de medidas de reconversión industrial y de incentivos para su desarrollo en aquellas áreas de futuro que se acomoden a nuestra capacidad geoeconómica de competir en este mundo global.

Estas medidas, por duras que resulten, pueden ser aceptadas por los ciudadanos. Los ciudadanos están dispuestos a arrimar el hombro, a colaborar con el nuevo gobierno para salir de esta situación, siempre que vea que se explican las cosas, que se cuenta con ellos, que no se miente y que no se hacen alianzas a sus espaldas. El ciudadano necesita creer en sus gobernantes, necesita seguridad jurídica, necesita ver que se protege su libertad y su dignidad y que se emplea adecuada y eficientemente el dinero que no es de nadie como decían, sino que es de todos nosotros. El ciudadano necesita una actividad ejemplar por parte de sus dirigentes y de los dirigentes de las grandes empresas, de los bancos y, en general de los dirigentes de las instituciones públicas y privadas del Estado.

El cambio del gobierno, el cambio del partido gobernante, es un rayo de esperanza, de esa ansiada esperanza tan necesitada. Confiamos en una nueva alternativa, no en una alternancia. Esperamos que esa luz que se vislumbra no se apague, que esa esperanza no se convierta en desencanto y, sobretodo no nos conduzca a la desesperanza. Estamos expectantes. Necesitamos ilusión, no nos decepcionen.”

COCIR – New kid on the block

The name COCIR is not the first name that comes to mind when looking at European healthcare. But that makes it’s findings and reporting on the state of healthcare in Europe – and its forecasts for the future – all the more important. In the world of accurate data, Content is everything, and the annual Report from COCIR on how Hospitals are going to spend their money – should be on your reading list.

As a Brussels based “Organisation”, the temptation is to think this is yet another US marketing organisation masquerading as healthcare. It’s key Members are as much American as European.  But not so.  We live in a global health market, after all. The COCIR focus is to develop European relationships,  including Russia and CIS areas, to offer a stable platform for the industry.

COCIR key findings

1. The European hospital health ICTs market totaled €2.5 billion in 2010 and is estimated to grow to €2.8 billion by 2015. (See 1.1)

2. The level of equipment of European hospitals in health ICTs varies greatly between types of systems and between countries. (See 1.2 and 2.3)

3. While hospitals are relatively well equipped with administrative information systems, they are insufficiently equipped with clinical information systems and show modest investment plans for these technologies which are crucial to improve healthcare delivery. (See 2.3 and 2.4)

COCIR recommendations

1. Collect evidence on the clinical and economic benefits of health ICTs

Given the low adoption of health ICTs by medical professionals, healthcare providers and payers, it is urgent to encourage adoption by demonstrating the benefits of these technologies both in clinical and economic terms. The shortage of studies documenting the economic benefits of health ICTs, clinical information systems in particular, is a challenge. Industry encourages governments and payers to finance such studies and to look into existing evidence when considering investments in health ICTs.

2. Build IT skills among healthcare professionals

Even when health ICT solutions are available in hospitals, healthcare professionals are not always equipped with the right set of IT skills to use those solutions. This can be explained ­ amongst other things ­ by resistance to innovative information technologies and a lack of time to learn how to use these new tools. This should be addressed by embedding IT skills in the medical curriculum and by providing IT training to healthcare professionals, including healthcare managers.

3. Invest in clinical information systems to increase efficiency of healthcare delivery

Clinical information systems are enabling tools: they increase the efficiency of healthcare delivery by supporting data transfer, workflow and decision­making. More investment in clinical information is needed to move today’s healthcare delivery models to the next level of efficiency and quality. Industry calls for more investment in order to move to integrated, more efficient, safer and patient­centered healthcare systems.1. HEALTH ICTs IN EUROPEAN HOSPITALS: GENERAL OVERVIEW OF THE

MARKET (2008­2015)

1.1 A MARKET WORTH €2.5 BILLION IN 2010

The European hospital health ICTs market totaled €2.5 billion in 2010. Moving forward, COCIR anticipates this market to reach €2.7 billion by 2015. These figures cover ICT solutions used in hospitals in Western and Eastern Europe: administration information systems, clinical information systems, laboratory information systems and imaging information systems (in the field of radiology and cardiology).

1.2 HEALTH ICTs: A VARIETY OF SYSTEMS SUPPORTING HOSPITALS’ MULTIPLE DEPARTMENTS AND FUNCTIONS

Health ICTs offer five types of information systems serving different purposes in hospitals:

Administrative information systems Market size in 2010: 940 M€. Estimated market size by 2015: 971 M€. Availability: 90% of hospitals are equipped with administrative IS. The market is saturated and moving towards a replacement cycle.

Cardiology IT (Cardiology Information Systems and Cardiology PACS) Market size in 2010: 55 M€. Estimated market size by 2015: 69 M€. Availability: Cardiology IT is still limited, with 10 to 30% of hospitals equipped. COCIR expects this small market to develop in the short­ to mid­term.

Clinical information systems Market size in 2010: 787 M€. Estimated market size by 2015: 936 M€. Availability: The availability of clinical IS is uneven, both between countries and between systems. The growth of this market is expected to be limited in the coming years.

Laboratory information systems Market size in 2010: 220 M€. Estimated market size by 2015: 242 M€. Availability: 80 to 100% of hospitals are fully equipped. The market is saturated and moving towards a replacement cycle.

Radiology IT (Radiology information systems and PACS) Market size in 2010: 533 M€. Estimated market size by 2015: 544 M€. Availability: The availability of radiology IT varies from country to country. UK and German hospitals are well equipped (70­95%), while France lags behind with 30% of hospitals equipped. 5% growth is expected in France over the period 2010­2015, perhaps more if national and regional plans under the “France sans film” programme are confirmed.

IHE Interoperability Testing

IHE-Europe launches first vendor-neutral service to test the interoperability of software used by healthcare information systems.

The new IHE-Services will help verify the ability of healthcare systems to work together, enhance the quality of software and improve the efficiency of systems in order to support the delivery of seamless patient care.

BRUSSELS, Belgium — Dec. 07, 2011 — IHE-Europe, a non-profit association dedicated to interoperability in health information and communication technology (HICT), today announced the creation of IHE-Services to offer professional interoperability testing services for health systems and projects.

In partnership with Kereval, an established software testing company for the telecom and automotive industries, IHE-Services will extend the tools and techniques developed for the annual IHE European Connectathon to provide interoperability testing for national e-health projects, HICT users in regional hospitals or vendors developing new platforms.

“The lack of interoperability between the systems used by different healthcare providers has been the greatest obstacle to a wider use of information technologies for better healthcare,” said Jacqueline Surugue, the Users Co-Chair for IHE-Europe.

“Offering a responsive service with access to the testing tools and expertise developed for IHE-Europe responds to a growing demand from e-health initiatives across Europe,” she said.

The creation of IHE-Services facilitates a technology transfer from INRIA, the French national institute for research in computer science and control, along with a number of INRIA engineers who will work on the IHE-Services team with Kereval.

The work pioneered by INRIA has been central to the growth and acceptance of IHE in Europe. This strategic evolution responds to the increasing number of requests received by IHE-Europe to provide vendor-neutral services for testing interoperability in e-health projects across Europe. Currently these tools are primarily used by participants at the annual IHE-Europe Connectathon event that brings together more than 300 software engineers from 90 organizations in an intensive connectivity marathon.

Succès de l’offre d’achat d’Ingenico sur XIRING

Suresnes, France, December 5, 2011: XIRING, a security solutions provider for electronic transactions, takes notice of the success of the contractual voluntary tender offer for its shares by Ingenico.

According to the information disclosed by NYSE Euronext, a total number of 3,459,123 XIRING shares have been tendered to the offer. Taking into account the 178,199 XIRING shares already held, Ingenico will hold a total number of 3,637,322 XIRING shares, representing 91.25% of the voting rights of XIRING on a non-diluted basis. The completion of the offer was subject to Ingenico’s holding at least 66.67% of XIRING’s voting rights on a fully diluted basis, which threshold has been by far crossed. The settlement date of the offer will be announced by NYSE Euronext.

In accordance with applicable regulation, Ingenico will file with the AMF, after settlement of the contractual voluntary tender offer, a project of simplified tender offer under the same terms and conditions for 17€ per share, for all XIRING shares not already held by Ingenico.
